Resource Community Outreach & Holistic Wellness Services, Inc. is a nonprofit, opportunity-based program designed to act for the Medicaid, Medicare, geriatric, Children & Adolescent population. RCOHWS's mission is to bridge the gap between individual needs and much-needed resources or services for daily functioning. RCOHWS helps save time, hassle, and undesirable stress by accurately connecting the community to different resources specific to their situational needs.
RCOHWS is designed to help connect the community to resources by promoting community tenure that may be holistically beneficial and support overall personal wellness. RCOHWS offers planned linkage and advocacy with several community providers to offer professional support for the member.
Our collaboration of services and resources is to provide a continuum of care individualized for each participant. RCOHWS works with the participant and the provider per a plan of action developed and approved by the participant and RCOHWS to reach their individual needs. By providing personalized care for the participant, we make it possible for them to remain successful in the community and in their daily lives.
